The Legal Aid Sentencing and Punishment of Young Offenders Act 2012 will change the name of Legal Services Commission to Legal Aid Agency on 1st April 2013. The Legal Aid Agency has amended all the forms listed to reflect this change of name. Applications for civil representation will only be accepted if they are submitted on the correct form. For example, the old LSC forms must be used for applications signed and dated on or before 31st March 2013. Applications which are signed and dated on or after 1st April must be submitted on the new and amended forms. Because any applications which are made on the wrong forms will be rejected, legal aid practitioners should check the Legal Aid Agency's website for more detailed information at www.justice.gov.uk/legal-aid on or after 1st April 2013. Any enquiries before 1st April should be directed at www.legalservices.gov.uk . Finally, the following forms have been made obsolete as part of these changes. CDS12, CLAIM1CHECKLIST, CLAIM2CHECKLIST, CLSADMIN3, CLSADMIN4, CLSAPP4, CLSAPP7, CSLCLAIM1(NOTES), CLSCLAIM1-APPENDIX, CLSCLAIM3, CLSCLAIM5(GUIDE), CLSCLAIM5(G) and CLSPOA1(GUIDE). These forms will automatically be deleted form your system(s).
