The Legal Services Commission has amended some of its existing forms and drafted new ones for use from 1st October.

CLSAPP1 and CLSAPP6 have been amended to reflect the introduction of the new Unified Contract for civil legal aid.

CLSAPP3, CLSAPP5 and CLSAPP8 have been amended to facilitate the introduction of Family Help.

CLSAPP7 has been amended to reflect a change to exemption for domestic abuse allegations and a new section has been inserted for completion by a mediator where mediation is unsuitable because of fear of domestic abuse.

CLSCLAIM1A is a new form for work conducted at level 3 of the Care Proceedings Graduated Fees Scheme.

CLSCLAIM4A has been introduced for payments made on account under the Care Proceedings Graduated Fees Scheme.

CWORK1 has been amended to cater for the introduction of Family Help.

CWORK1PL is a new form for family lawyers advising parents where a local authority has served them notice under section 31 of the Children Act 1989.

CWORK2 has been amended to reflect new disability monitoring and the unified contract specification and CWORK2(IM) has been revised to cater for the Immigration Specification.

CWORK3A(IM) and CWORK3B(IM) are new forms to ensure a continuing facility for legal aid practitioners to apply for extensions to the Upper Costs Limits in those cases that are not covered by the new Graduated Fee Scheme which will come into force on 1st October. These two forms effectively replace the current form CWORK3(IM) which therefore needs to be deleted from your system(s).

CWORK3C(IM) is a new form to enable practitioners to apply for an extension to the Disbursement Limit for cases funded under the Graduated Fee Scheme.

CWORK4 has been amended to reflect the requirements of the new Immigration Specification and CWORK4(MH) is a new form to cater for the introduction of Family Help.

The Legal Services Commission has added a field to the CMSF form to assist in processing and updating the contract number.

CDS6 has been amended to include a Police Station Identifier field. A revised list of Police Stations and Police Station Schemes will appear on the Legal Services website prior to the mandatory date.

The Legal Services Commission has drafted separate CMRF forms for the new Family Help Scheme, Immigration, Mental Health and TFF matters [CMRF(IMM), CMRF(FH), CMRF(MH), CMRF(TFF)]. These four new forms replace the existing CMRF form which can only be used until 10th October for September’s submissions. Therefore, form CMRF also needs to be deleted from your system(s).

Clause 15.2 of the Unified Contract Standard Terms 2007 requires all Controlled Work claims to be submitted via the Legal Services Commission’s website. The Consolidated Matter Report Forms CMRF(IMM), CMRF(FH), CMRF(MH), CMRF(TFF) and Controlled Matter Start Form CMSF should therefore be made and submitted via the Legal Services Commission’s electronic billing system. The Legal Services Commission will only accept CMRF(IMM), CMRF(FH), CMRF(MH), CMRF(TFF) AND CMSF in paper format in “exceptional circumstances” as the Legal Services Commission expects these forms to be filed via its website.


Finally, EC-CLAIM1 is a new form where a case is ‘flagged’ as exceptional after any CMRF forms or form CMSF has been submitted.

None of these new and amended forms should be used before 1st October.
