HMCTS have updated the listed form which is now available for immediate use. Changes include:

• Added guidance about what to do if you do not have a legal representative.


• Added a question asking which court heard the case.


• Added guidance on what to do if service was affected by email or other electronic means.


• Added a 'Statement of truth' section.
