The Family Proceedings (Amendment) (No. 5) Rules 2005 have amended the above forms to take account of the creation of civil partnerships under the provisions of the Civil Partnership Act 2004 on 5th December.

Minor amendments have also been made to the FL415, which is available for your use.

The amended versions of FL401, FLGN, FL402 and FL416 cannot be used before 5th December.
