EP2PG - Application for Registration of an Enduring Power of Attorney

The Office of the Public Guardian has amended various details on the forms listed to refer to a new postal address, telephone numbers and website addresses to download information which will change on 1st October.

Practitioners should continue to use current versions of all these forms until 30th September. The amended forms should not be used until on or after 1st October.

OPG120A is a new form and LPA120A replaces OPG506A for applications made on or after 1st October. OPG506A will automatically be removed from your system(s).

Finally, please find forms COP5 and COP20A which have received slight amendments which can be used immediately.
