EFIS 36, June 2009 : Court of Protection

On 5th May, the Office of the Public Guardian required all postal applications to be sent to the Office of the Public Guardian, PO Box 15118, Birmingham, B16 6GX instead of its London office.

We have amended the forms listed to take account of this change of address, except for form COP5 which has received a minor field amendment.

All forms should be immediately.
