PI_LIT14 - Form of consent from a third party to release physiotherapy records

Oyez has amended all the forms PI_LIT1 to PI_LIT22 in this range to refer to the Data Protection Act 2018 where these forms previously referred to the old Data Protection Act 1998.

According to the Parliamentary website at:-

the Data Protection Bill is expected to receive Royal Assent on 23rd May.
