The Legal Services Commission have amended the above forms to take account of new guidance to the Funding Code and must be used from 1st April. Practitioners should note that CLSADMIN5 and CLSCLAIM5 are new forms which have been introduced by the Legal Services Commission to support the Family Graduated Fee Scheme. CLSADMIN5 and CLSCLAIM5 should only be used by solicitors with a Family Franchise and ought not to be used before 1st May when the Family Graduated Fee Scheme is introduced. Please delete your existing versions of CRIMAPP1 and CRIMAPP2 which will become obsolete from 1st April due to the introduction of Criminal Contracting as well as the Pilot block contracting forms numbered CCR1-8 which were supplied to you on EFIS 15 in July 1998. ~ NEW CRIMINAL DEFENCE SERVICE FORMS CDS1 - Client's Details Form CDS2 - Application for Advice and Assistance CDS3 - Application for Advocacy Assistance CDS4 - Application for Prior Authority to Incur Disbursements In Criminal Cases CDS5 - Application for Extension of Upper Limited CDS6 - Contract Work Report Form CDS7 - Non_Standard Fee Contract Work Assessment Form CDS8 - Assigned Counsel's Fee Note in Criminal Cases CDS9 - Police Station Standby Report CDS10 - Application for Review or Refusal or Withdrawal of Advocacy Assistance or Representation CDS11 - Generic Costs Form ~ The forms listed above have been drafted by the Legal Services Commission to support the General Criminal Contract which will be introduced on 2nd April. None of these new forms should be completed and submitted until your firm starts new work on or after 2nd April under the terms of the General Criminal Contract. The table below seeks to equate the new forms to your current forms. New Criminal Forms Forms A and B will replace your existing Criminal Forms 1 and 5 when the Lord Chancellor's Department have finalised Criminal Forms A and B. We will supply these in due course. Where your firm wishes to submit a claim for work begun before 2nd April, you should use the old forms provided they reach the Legal Services Commission by 10th April.
