Further to EFIS No. 5 which we released in March, the forms listed above have been revised
to cater for comments made by certain district Probate registries.

The improved forms are included on the enclosed disk and should be used immediately on
receipt.

We have also taken this opportunity to amend those Probate forms which have required
alteration to accommodate the Millenium date changed. The reference to ‘19’ has been
deleted and the relevant filed left blank for users to complete, as appropriate. The other
ranges of Oyez forms on disk will be similarly revised at various points through the reminder of this year.
