Subscribers to this range are requested to use the latest versions of the Lasting Power of Attorney forms listed after the Office of the Public Guardian made minor corrections to these new forms after Oyez supplied them to you on EFIS 39/2015 in June.

Please also find available OPG120A and OPG120(NOTES) which have also been updated.
