Following discussions with the British Medical Association (BMA), the Law Society has added a page of information to the front of this form concerning the use of the client's health records.

In addition, the Law Society has also amended LSCF to amend the old statutory references to the Data Protection Act 1998 to refer to the new Data Protection Act 2018 which came into force on 25th May 2018.

Personal injury practitioners should therefore use the latest version of this form immediately.
