Following the recent release of Companies House's (CH) changes to this form on EFIS No. 60 last month, Companies House has notified Oyez of an amendment which CH wants to make to this form.

Because the redacted barcode on page 2 for "usual residential address" was not being recognised by the scanners at CH, CH has corrected this fault and has asked Oyez to reissue PSC04 to avoid this problem recurring in the future.

Subscribers to this range are therefore requested to use this latest version of PSC04 immediately.

Please also find enclosed PSC01, PSC05, PSC06, PSC07, PSC08, PSC09 and CS01_Combined which all received minor amendments.

All forms can be used immediately.
