Following feedback from the scanning department of the Office of the Public Guardian we have amended forms LP1F, LP1H, LP2 and LPC.

These forms are now fully compliant with the OPG's scanning requirements. There were no amendments to fields in these forms.

Please also find available forms COP1 form which has received a slight field amendment on page 4, where box 4.1 was expanded and OPG102 which has received a minor field amendment.

All forms should be used immediately.
