The Office of the Public Guardian (OPG) has been working on improving the deputy report forms for both property and financial matters and health and welfare decisions by making their new forms easier to understand.
The OPG will replace the current combined deputy report form on 1st March with a new OPG102 form for property and financial decisions and a new OPG104 form for health and welfare decisions.
The OPG will also introduce a shorter version of OPG102 which will be numbered OPG103. New and existing Court appointed deputies will be asked to complete OPG103 when they submit their first report.
Depending on the type of their Court order, all other deputies will be asked to complete either OPG102 or OPG104. Deputies with both types of order will have to complete the OPG102 (or OPG103) and OPG104 each year.
In future, deputies will have to answer additional questions on OPG102 such as the level of contact with their clients, the funding of their client's care and whether or not the client is receiving all the benefits to which they are entitled. More detailed information will also be sought in relation to money received and money paid out. In addition, specific questions are raised on OPG104 concerning the client's care arrangements, health and social activities. In future, deputies will also be asked to inform OPG of any significant decisions which they plan to make over the next twelve months.
Professional and public authorities will also be asked for more information about their fees and charges. Professional deputies should use OPG105 to provide information about their fees and public authorities should use OPG106 about their fees and charges.
The OPG will continue to accept either the existing version of OPG102 or the new version of OPG102 until 1st September. However, old forms which are signed and dated after 1st September will not be accepted after this date when only the new versions of OPG102 and OPG104 should be used.
