Pursuant to the information provided to you on EFIS 12/2015 in February and on EFIS 26/2015 in April, subscribers to this range are requested to use these four new EEA forms, together with the latest version of the amended AN form.

As previously advised, EEA(EFM) replaces the old EEA1 form. The new EEA(FM) form replaces EEA2 and the new EEA(PR) form performs the same function as the old EEA3 and EEA4 forms.

EEA(PR) is a new application form for use by an EEA or Swiss national who is in the UK as a qualified person.

All forms can be used immediately.

EEA(QP)	Application for a registration certificate as a European Economic Area (EEA) or Swiss national who is in the UK as a qualified person	New/Amended
