The Ministry of Justice has amended form FL401a to reflect changes which the Crime and Courts Act 2013 and the Children and Families Act 2014 will introduce on 22nd April.

Please also find available FL403 which has received a slight field amendment and can be used immediately.

We understand further changes to Family Law forms may be made by the MoJ shortly. Should this be the case we will make these forms available as soon as possible.
