Following the publication of these new and amended Children Act forms on Efis No.27/2011 earlier this month, the Ministry of Justice has made some minor amendments to the layout and wording of C2, C2-Flexi , C67, C68, C69, C100 and DIV_E2.

All forms should be used immediately.
