The Legal Services Commission has made a variety of miscellaneous amendments to the forms listed which should be used from Monday 4th April.

AF2 is a new form, which can be used to request a review of the fee paid by the Advocates Graduated Fee Team.

The Legal Services Commission has amended the references on CDS14, CDS15 and CDS16 which refer to HM Courts Service (HMCTS) and made other adjustments to the text.

The field for accreditation PIN number has been removed from the bottom of page 7 of CWORK2(IM) and a field for �PA Ref� has been added to CWORK3A(IM), CWORK3B(IM) and CWORK3C(IM).

The Legal Services Commission has made extensive changes to the AF1 and minor alterations to the content of the CLSCK3 and LF1 forms.
