Her Majesty�s Court Service has amended the Court of Protection forms listed to refer to new contact details for the Court Enquiry Service and to refer to a new website address of www.direct.gov.uk

Although the Court of Protection will continue to accept the existing editions of forms COP1-COP36, practitioners may prefer to use the latest versions of these forms.

We have also taken this opportunity to add COP12 to this range of forms.

Finally EP1PG(3) has received minor amendments.

All forms can be used immediately.
