EFIS 15, March 2010 : Adoption

The Ministry of Justice has created two new Adoption forms to be used on or after the 6th April 2010.

A64A is a new application form to obtain information from court records about a parental order under section 60(4) of the Adoption and Children Act 2002.

A101A is a new form of agreement to a parental order being made under section 84 of the Human Fertilisation and Embryology Act 2008.

Both of these forms should not be used before the 6th April 2010.

A101A	Agreement to the making of a parental order in respect of my child		New/Amended
