The Family Proceedings (Amendment) Rules 2010 and the Family Proceedings Courts (Children Act 1989) (Amendment) Rules 2010 prescribe a new Children Act Form C110 for use when applying for a care or supervision order and make consequential changes to existing Children Act form C1 to reflect these amendments.

This new Children Act form C110 replaces the current Children Act form C13 which should no longer be used after 6th April. Please see deletions below.

The new Children Act forms C51 and C52 have been created for use under section 54 of the Human Fertilisation and Embryology Act 2008. C51 is an application for a parental order and C52 is the response to such an order.

Since the Family Proceedings (Amendment) Rules 2010 will not come into force until 6th April, none of these new or amended Children Act forms should be used before this date.
