The Court of Protection forms listed have been amended to refer to Her Majesty�s Court Service website after the Court of Protection became part of HMCS.
Although the Court of Protection will continue to accept the 2007 editions of these forms, practitioners are requested to use these amended forms, which refer to the current website address.
