The Human Fertilisation and Embryology Act 2009 will come into force on 1st September.

The Ministry of Justice has amended Adoption forms A58, A60 and A61 to comply with these new statutory provisions.

Current editions of these forms can continue to be used until 31st August. The amended forms must be submitted in applications made on or after 1st September.

Forms affected

A58	Application for an adoption order section 46 Adoption and Children Act 2002	New/Amended
A60	Application for an adoption order (excluding a Convention adoption order)	New/Amended
A61	Application for an order for parental responsibility prior to adoption abroad	New/Amended
