The Legal Services Commission has amended these forms to take account of changes which will be introduced on 1st June.

The new CLSPOA1 form replaces the current CLSCLAIM4 and CLSCLAIM4A and the amended CLSAPP6 form replaces the existing CLSMEANS6. Therefore, CLSCLAIM4, CLSCLAIM4A, CLSCLAIM4A(NOTES) and CLSMEANS6 need to be removed from your systems.

All forms can be used immediately, but they must be used from 1st June.

If you are not using Active Library, you will need to manually delete some forms. For deletion instructions, click here
