This form should be used to create an arrangement under the Adults with Incapacity (Scotland) Act 2000 which allows someone to continue to pay bills and meet regular expenses for a person who can no longer do that for themselves. It is sometimes known as an “Authority to Intromit with Funds.”

This form can be used immediately upon receipt.
