The Public Guardianship Office has recently revised form CP5 to include an ethnicity questionnaire for monitoring purposes at the back of the form and has also taken the opportunity to make minor alterations to CP5.

The existing version of CP5 can continue to be used but it is recommended that practitioners load this latest version of CP5 onto their system upon receipt of this update.
