The Court Service has updated the above form by expanding both the methods for service and the details of the address for service. The amended form takes effect from 1st June 2004.

There have also been minor amendments to the N16A, N245 and N379, which are available for your immediate use.

The CPR.MDB file, which contains court address details, has been updated and is available from the Support section within this website.

Please download the CPR.MDB file from this website and save to the location as specified in Tools > Options > Database in your OyezForms program.
