The Court Service has made amendments to forms N245 and N251.

The N251 has been amended to include insurance policy numbers and insurers address. Also, the notes for form N245 have been amended.

Finally, minor textural amendments have been made to the N293A, which is also enclosed.

All forms can be used immediately upon receipt.
