The Court of Protection (Enduring Powers of Attorney) Rules 2001 have re-prescribed EP forms 1-4 to take account of the fact that these applications can only now be made to the Court of Protection (whereas the current forms refer to the Public Trust Office as well). Please use the amended versions of EP1-4 from 1st April.

The Court of Protection Rules 2001 prescribe the Court of Protection Forms A, B and C for use from 1st April.

New Court of Protection form A replaces your current CP9, whilst Forms B and C are new Court of Protection Forms. Instructions for removing CP9 are enclosed.

Court of Protection Forms A, B and C should be used from 1st April.
