By a Direction dated 12th January, the Senior District Judge of the Principal Registry of the
Family Division of the High Court decreed that in future the name, dates of birth and death
of the deceased be inserted on the forms listed above where death occurred in the United
Kingdom and the death has been recorded in the Register of Deaths.

The revised forms are included on the enclosed disk and should be used immediately on
receipt.
