HMCTS has redesigned the following grant of representation forms for use by the legal sector. The forms must be completed in place of the statement of truth. The forms were introduced on 23rd March 2020. For a period of 8 weeks, professionals will complete the forms, lodging them at registries as normal. The registries they will need to send their forms to will be signposted by a new temporary probate-professional only PA4 Sot. During this period, statements of truth will still be accepted, until 18th May 2020. The updated forms are now available for use.

Court Statements of Truth – Information

The 113th Civil Procedure Rule update relating to PD 22 – Statements of Truth, came into force on 06 April 2020. A large number of litigation forms will require amendment to reflect the new wording however these forms are subject to Crown copyright. As a result we cannot change the forms until the official versions are updated. We are in contact with the relevant agencies and are assured that these forms are currently being reviewed. As soon as we are able to amend the forms we will do so as a matter of urgency.
