Following the recent release of Companies House's (CH's) changes to this form on EFIS No.59 last month, Companies House notified Oyez of an amendment which CH wants to make to this form.

Because the redacted barcode on page 2 for "usual residential address" was not being recognised by CH's scanners, CH has corrected this fault and has asked Oyez to reissue LLPSC04 to avoid this problem recurring in the future.

Subscribers to this range are therefore requested to use this latest version of LLPSC04 immediately.
