The Court Service has drafted new forms to enable applications to court to disqualify company directors under the Company Directors Disqualification Act 1986. These new forms cannot be used before 1st October.

N500 is the basic claim form against a company director. N500a is a set of notes which help the Claimant to complete the form and N500b are the set of notes which help the defendant to reply to the allegations made in N500.

N502 is the Defendant�s response to N500. N503 is the Defendant�s response to N502.

N501 is a claim under section 8A of the 1986 Act where an individual voluntarily agrees not to act as a company director.

N504 is the pre-trial checklist which must be completed before proceedings involving the disqualification of company directors can be heard.
